Designed for new or returning patients with one to two primary health concerns.

 

This visit includes a focused health history, physical examination (when appropriate), a comprehensive medication review, and the development of an individualized care plan. Virtual or In-Person Available.

Follow-up appointments provide ongoing care for:
 

• Bloodwork review

• Diagnostic imaging results (X-rays, ultrasounds, CT scans, and MRIs)

• Medication monitoring and adjustments

• Acute illness follow-up

• Chronic disease management

• Continued treatment planning

Medication refills are available for prescriptions previously provided through our clinic. Virtual or in-person appointments available,

ADHD Medication Titration
 

For clients with a confirmed ADHD diagnosis, this appointment focuses on safely starting or adjusting medication to determine the most effective dose. Medication titration is a gradual process and may require follow-up visits to monitor progress and response.

ADHD Post-Step 3 Check-In
 

This follow-up appointment is designed for clients who have completed a Step 3 ADHD Assessment. During this visit, your provider will:

• Monitor symptom improvement

• Review medication effectiveness

• Assess for side effects

• Discuss counselling or therapy supports

• Make medication adjustments if appropriate
   

ADHD Diagnosis Review & Re-Assessment
 

If you have previously been diagnosed with ADHD outside Flourish Health & Wellness or The Suites, this appointment includes a review of your previous assessment, confirmation of diagnosis when appropriate, an updated assessment if needed, and personalized treatment planning.

Weight Management Assessment
 

Explore personalized, evidence-based options for achieving your health and wellness goals. During this assessment, you'll discuss lifestyle strategies, nutrition, medical treatment options, and individualized plans to support healthy, sustainable weight management.

Completion of medical documentation, including:

• Motor Registration forms

• Insurance documentation

• Pre-authorization forms

Off Work Medical Note
 

Medical documentation for short-term absence from work due to an acute illness.

Please note: Long-term disability or extended sick leave documentation is not provided through this service.
 

Disability Tax Credit (DTC)

A comprehensive medical assessment and completion of the required documentation to support eligibility for the Disability Tax Credit (DTC).
